Sick Certificate

A Medical Illness Certificate (Sick Note) is often required by employers, schools, or institutions to confirm that you are unfit for work or study due to illness. At Dr Clinic, this certificate can only be issued following a general consultation, where our doctor will carefully assess your overall health, review your symptoms, and determine whether rest or time away from work is necessary. This ensures that the certificate is accurate, medically valid, and tailored to your individual situation. Our goal is to provide you with the proper documentation while supporting your recovery and helping you return to your daily activities safely and confidently.

Note: Any changes in dates or extensions to medical/sick certificates, as well as patient letters, will incur an administration fee of €10. Referral letters are not included in this fee.

Employment Forms & Fitness to Work Certificates

At Dr.Clinic, we provide Fitness to Work Certificates and assist with the completion of various employment-related medical forms required by employers or occupational health departments. These may include pre-employment assessments, return-to-work certificates following illness or injury, and forms related to workplace health declarations. During your consultation, the doctor will carry out a focused clinical assessment to evaluate your current health status and determine your suitability to return to work or fulfil specific job requirements. Please note that booking a consultation does not guarantee that a certificate or form will be signed, as this is a clinical decision based on the doctor’s professional judgment. If the doctor determines that you are not fit for work at the time of consultation, or that further evaluation is needed, they will advise you accordingly and may recommend follow-up with your regular GP or specialist. Patients are also advised to bring any relevant medical documentation or previous reports to support their assessment. Dr.Clinic does not provide long-term occupational health follow-up; our role is to perform a single-point clinical evaluation and provide documentation only when deemed appropriate and safe in the best interest of the patient.

Referrals & Medical Letters

At Dr.Clinic, we provide a range of medical referrals and supporting letters following a clinical assessment, when deemed appropriate by the attending doctor. These may include referrals for diagnostic imaging such as MRI, X-ray, or ultrasound scans, as well as specialist consultations in both medical and surgical fields. We also offer referral letters for the Emergency Department (ED) in cases where urgent face-to-face care or further evaluation is required. In addition, we can provide travel-related medical letters, such as documentation to certify that a patient is carrying prescribed medications or medical devices when flying.
Please note that all referrals and letters are issued at the discretion of the doctor following a medical consultation and are based on your current condition and clinical findings. Booking a consultation does not guarantee that a referral or letter will be issued. For chronic or complex health conditions, we may recommend follow-up with your regular GP or specialist for ongoing care.
Referral and Letter Services We Provide:

Driving License NDLS Form

At Dr.Clinic, we assist patients with NDLS medical assessments required for driving licence applications or renewals. Our doctors are registered to complete and certify NDLS Medical Report Forms following a consultation and health review.

According to NDLS guidelines, individuals with certain ongoing or complex medical conditions — such as epilepsy, insulin-treated diabetes, neurological disorders, visual impairments, psychiatric illnesses, or those requiring long-term monitoring — are advised to have their medical forms completed by their regular GP or specialist who has full access to their medical history. In such cases, a walk-in clinic may not be a suitable setting for form completion.
This ensures that all assessments are conducted safely and in the best interest of the patient.

Social Welfare Certificates

All forms, including NDLS Medical Report Forms and Fitness to Work Certificates/employment forms, are subject to a clinical assessment by the attending doctor. If the doctor determines that the form cannot be completed, they will advise you on the appropriate next steps, which may include referral to your regular GP or specialist.

If you have a private insurance you may request a headed receipt after completing your payment and submit your receipt to your insurance provider for the reimbursement.
